I understand that if I develop depression, suicidal thoughts, or sexual dysfunction, I must stop treatment immediately and seek urgent medical advice.
I understand that sexual dysfunction may, in rare cases, persist even after stopping finasteride.
I will seek medical review if I notice changes in my breast tissue, including lumps, pain, or nipple discharge.
I understand that finasteride can affect Prostate Specific Antigen (PSA) test results, and I will inform my doctor that I am taking this medication before having this test.
I will read the patient information leaflet supplied with my medication.
I will contact you and inform my GP if I experience any side effects, start new medication, or if my medical conditions change during treatment.
I confirm that this treatment is solely for my own use.
I confirm that I have answered all the above questions accurately and truthfully. I understand that prescribers base their decisions on my responses in good faith, and that providing false or incomplete information may be hazardous to my health.
