How It Works

Orforglipron is a non-peptide GLP-1 receptor agonist. It binds to and activates GLP-1 receptors, including those in areas of the brain involved in appetite regulation, which reduces food intake.

It also improves blood sugar control by increasing glucose-dependent insulin secretion, reducing fasting glucagon, and improving insulin sensitivity. It delays gastric emptying; this effect is largest after the first dose and lessens over time.

Because orforglipron is a small molecule rather than a peptide, it is stable in the digestive tract without an absorption enhancer, which is why there are no fasting, water or timing restrictions.

Administration: Take one tablet by mouth once daily, at any time of day, with or without food. Swallow whole — do not break, crush or chew. Do not take more than one tablet per day.

Dose titration: Start at 0.8 mg once daily for 30 days, then increase to 2.5 mg once daily. The dose may then be increased in steps to 5.5 mg, 9 mg, 14.5 mg and 17.2 mg, remaining at each dose for at least 30 days before any increase, based on response and tolerability. The maximum dose is 17.2 mg once daily. Do not take more than one tablet to achieve a higher dose.

Missed dose: Take it as soon as you remember. Do not take more than one dose in a day, and do not double up.
Stopping: Do not stop taking Foundayo without speaking to your prescriber.

Serious allergic reaction (anaphylaxis) — breathing difficulty, swelling of the face or throat, wheezing, fast heartbeat, pale cold skin, dizziness or weakness. Get immediate medical help.
Increased chance of food or liquid entering the lungs during surgery or procedures
Kidney problems, sometimes requiring dialysis

Warnings

Allergies: Do not take Foundayo if you are allergic to orforglipron or any of the other ingredients.

Pancreatitis: Foundayo has not been studied in people with a history of pancreatitis and should be used with caution. Tell your prescriber if you have ever had it.

Low blood sugar: If you take a sulfonylurea or insulin, your risk of hypoglycaemia is increased. Your prescriber may reduce the dose of those medicines, and you should monitor your blood glucose as instructed.

Digestive conditions: Tell your prescriber if you have severe digestive problems, including severe gastroparesis (delayed stomach emptying).

Dehydration and kidneys: Nausea, vomiting and diarrhoea can lead to dehydration, which may affect kidney function. Drink enough fluids, particularly if you have existing kidney problems.

Blood pressure: Foundayo may lower blood pressure. Tell your prescriber if you take medicines for high blood pressure.

Gallbladder: Tell your prescriber if you develop upper abdominal pain, yellowing of the skin or eyes, pale stools or fever.

Eyes: Tell your prescriber if you have diabetic retinopathy or macular oedema.

Heart failure: Foundayo is not recommended if you have severe (NYHA class IV) heart failure.

Surgery and anaesthesia: Tell your surgical team and anaesthetist that you are taking Foundayo before any procedure involving general anaesthesia or sedation, as GLP-1 medicines delay stomach emptying. Do not stop treatment on your own — the decision is for your prescriber and anaesthetist.

Liver: Not recommended in severe liver impairment.

Children and adolescents: Not for use under 18 years.

Oral contraceptives: Foundayo may reduce the effectiveness of oral hormonal contraception. If you use the oral contraceptive pill, switch to a non-oral method or add a barrier method (such as condoms) for 30 days after starting Foundayo and for 30 days after every dose increase.

Pregnancy and breastfeeding: Do not use during pregnancy or while breastfeeding. Use effective contraception during treatment. If you are planning a pregnancy, Foundayo should be stopped at least 3 weeks beforehand.

Other medicines: Tell your prescriber about all medicines you take. Interactions include clarithromycin, ketoconazole, itraconazole, ritonavir and telaprevir (which raise orforglipron levels); carbamazepine, phenytoin, rifampicin, bosentan, efavirenz and St John’s wort (which lower it); and simvastatin, rosuvastatin and topotecan (whose levels orforglipron may raise). Dose limits apply with some of these — your prescriber will advise.
