Why side effects happen — and why they ease

Semaglutide slows how quickly your stomach empties and reduces appetite. Early on, that can cause nausea and other gut symptoms while your body adapts. This is exactly why the pill is titrated — started at a low 1.5mg dose and increased gradually — so your system adjusts step by step rather than all at once. Practical tips to minimise side effects

  • Take the tablet correctly — empty stomach, ≤120ml water, wait 30 minutes. Correct dosing also aids consistency.
  • Eat smaller portions — your appetite is reduced, so large meals often trigger nausea.
  • Go easy on fatty and sugary foods — the most common nausea triggers.
  • Stay hydrated — especially with diarrhoea or vomiting.
  • Don’t rush dose increases — your prescriber can hold you at a dose for longer if it’s hard to tolerate.

Side effects after a dose step-up

Each time you move up a strength, expect a short return of early symptoms — typically nausea for a few days — as your body adjusts to the higher dose. This is normal and usually settles within a week or two. If it doesn’t, or if it’s severe, your prescriber can slow your titration. When to seek urgent help

Seek urgent medical care if you experience severe abdominal pain that spreads to your back (a possible sign of pancreatitis), persistent vomiting with inability to keep fluids down, signs of an allergic reaction (swelling, difficulty breathing — call 999), or gallbladder symptoms (pain in the upper right abdomen, fever, yellowing of the skin or eyes). These are uncommon but need prompt attention.
